TRAUMATIC BRAIN INJURY PROGRAM | FAMILIES FIRST

Provides case/care management, supported employment services that include job coaching and placement, social and recreational opportunities and ongoing assessment and evaluation. May also provide assistance with the activities of daily living, including personal care, respite care, and transportation. Also provides Medicaid Waiver, a community-based, one-to-one, long term care services program to individuals with traumatic brain injury. Referrals may come from Department of Aging & Independent Living; Services are offered to brain injured survivors following their post-acute care, and have a finite time limit, depending on need and severity of injury. Families First is a Specialized Service Agency, designated by Vermont's Department of Aging and Independent Living.

INTELLECTUAL AND DEVELOPMENTAL DISABILITY SERVICES | UNITED COUNSELING SERVICE OF BENNINGTON COUNTY

Provides comprehensive services to individuals with developmental disabilities, autism, and other pervasive developmental disorders. Individuals are assigned professional service coordinators who are responsible for clients' Individualized Support Agreements, and for assisting them in achieving success in the community. Services include personal support and case management; specialized residential programs; psychiatric and medical care; vocational and skill acquisition training; family support services; and support services to individuals in nursing homes. Provides a spectrum of supported living arrangements to meet the needs of each person. HOSPICE CARE VOLUNTEER SERVICES | CENTER FOR SOLACE

Provides trained volunteers who offer non-medical assistance to hospice patients and their families. Assistance includes companionship, personal care, assistance with daily living, transportation. Additional services include provision of vigil sitters who provide a loving presence during the last days of life, respite for caregivers, and bereavement follow-up support. Program is for people with progressive endstage illness and a prognosis of up to two years. Clients may choose to continue aggressive medical therapies.
