LONG TERM CARE SERVICES | CENTRAL VERMONT HOME HEALTH AND HOSPICE
Provides comprehensive, coordinated long-term care in an individual's home. Services include case/care management, homemaker assistance and assistance with the activities of daily living, including personal care and respite care that enables frail elderly and permanently disabled individuals to live safely, comfortably and as independently as possible at home. Also provides Medicaid Waiver, community-based one-to-one long term care services to individuals with Traumatic Brain Injury. Referrals may be made by physicians, hospitals, community agencies, patients themselves, or family members
