LONG TERM CARE SERVICES | VISITING NURSE ASSOCIATION AND HOSPICE OF THE SOUTHWEST REGION
Provides comprehensive and coordinated long term care in an individual's home. Services include case/care management, homemaker assistance, and assistance with the activities of daily living. Also provides Medicaid Waiver, community-based one-to-one long term care services to individuals with Traumatic Brain Injury (TBI). Referrals may be made by physicians, hospitals, community agencies, patients themselves, or family members.

The Visiting Nurse Association & Hospice of the Southwest Region is a non-profit, Medicare-certified, home health care and hospice agency delivering services to residents of Southwestern Vermont and Franklin County. Care is provided to adults who need in-home assistance, rehabilitation, long term care, or end-of-life care, as well as to critically ill children and to vulnerable young families. The Agency promotes the general welfare of residents through health promotion outreach and community wellness programs. Services include Home Care, Hospice Care, Palliative Care, Long Term Care, Rehabilitative Therapies, Maternal Child Health Services, Pediatric Nursing, and Private Duty Nursing Services.
