LONG TERM CARE SERVICES | VISITING NURSE AND HOSPICE FOR VERMONT AND NEW HAMPSHIRE
Provides comprehensive and coordinated long-term care in an individual's home. Services include case/care management, homemaker assistance and assistance with the activities of daily living. Also provides Medicaid Waiver, community-based one-to-one long term care services to individuals with Traumatic Brain Injury (TBI). Referrals may be made by physicians, hospitals, community agencies, patients themselves, or family members.

Visiting Nurse and Hospice for Vermont and New Hampshire (VNH) is a non-profit, Medicare-certified, home health care agency delivering services to residents of southeastern Vermont and southwestern New Hampshire. Care is provided to adults who need in-home assistance, nursing, rehabilitation, long term care, adult day services or end-of-life care, as well as to critically ill children and to vulnerable young families. VNH also encourages the general welfare of residents through health promotion outreach and community wellness programs. Services include Home Care, Hospice, Palliative Care, Long Term Care, Rehabilitative Therapies, Maternal Child Health Services, Pediatric Nursing, Pediatric Palliative Care, and Private Duty Services.
