LONG TERM CARE SERVICES | BAYADA
Provides comprehensive and coordinated long-term care in an individual's home. Services include case/care management, homemaker assistance and assistance with the activities of daily living. Also provides Medicaid Waiver, community-based one-to-one long term care services to individuals with Traumatic Brain Injury (TBI). Referrals may be made by physicians, hospitals, community agencies, patients themselves, or family members.

Bayada is a Medicare-certified, for-profit home health care agency that provides skilled nursing, physical therapy, occupational therapy, speech therapy, and social work services for adults and children throughout the state of Vermont. Bayada is also a designated provider of Vermont's Choices for Care, a long term home health care program and Vermont's Traumatic Brain Injury Program (TBI Waiver) provider for community support services of individuals recovering from traumatic brain injury.
