Description

Provides comprehensive and coordinated long-term care in an individual's home. Services include case/care management, homemaker assistance and assistance with the activities of daily living. Also provides Medicaid Waiver, community-based one-to-one long term care services to individuals with Traumatic Brain Injury (TBI). Referrals may be made by physicians, hospitals, community agencies, patients, or family members.
