Provides monthly supplemental nutrition assistance benefits to low-income households to purchase food. Households in which any person is younger than 65 access their benefit using a Vermont Express electronic benefits transfer (EBT) debit card. If everyone in a household is 65 or older, or receiving Supplemental Security Income (SSI), benefits are deposited as cash into the recipient's bank account. Expedited benefits are available in emergencies and other specific situations. EBT cards may also be used to buy fresh, local products at certain farmers' markets in Vermont. A list of farmer's markets that can accept EBT cards is available on the website. In addition, 3SquaresVT benefits may be used for seeds and plants that grow fruit and vegetables at any retailer or farmer's market that accepts EBT cards.

Provides individualized, comprehensive outpatient services for the treatment of opioid use disorders with medication and recovery supports. Treatment with suboxone or vivitrol; suboxone provider support, education, and consultation; case consultation around the interface between pain and opioid use disorders; and group and/or individual therapy to help patients in rebuilding lives damaged by opioid use disorder. Also provides treatment of alcohol use disorders with counseling and medications such as vivitrol, antabuse, or campral.

Provides disposable diapers and wipes, as well as adult incontinence supplies.

Provides a comprehensive online listing of assisted living facilities in Vermont. Assisted Living Residences are state licensed residences that combine housing, health, and supportive services to support resident independence and aging in place.

Provides welcome visits upon the arrival of a new child to the family, including information on community resources, health education and services, safety, and parenting.

Provides monthly cash assistance to low-income households in which a person's care or services are essential to an elderly or disabled person's remaining in his or her home. Benefit can be directly deposited into a bank account, or put onto an EBT card.

Sponsors a weekly community supper. Also provides a Christmas lunch on Christmas Day.

Assists with personal needs items and incidentals expense assistance as well as emergency dental, medical, and prescription assistance, medical supplies and equipment assistance, and burial costs.

Offers adult education and literacy instruction that provide individuals with the opportunity to acquire the education and literacy skills they need either to complete a secondary education or to better achieve post-secondary/career goals. Instruction is provided at six learning centers, as well as at job sites, libraries, homes, and other locations. Services include Functional Literacy Education (Reading, Writing, Math), Computer Literacy, Financial Literacy, English Language Learning and Citizenship Preparation, High School Completion Program, General Education Development (GED) Credentialing, Career and College Readiness, and other certifications.

Provides internet-based workshops and events for job seekers. Offerings include Interview Skills, Re-Employment Strategies, Virtual Job Fairs, and Regional Employment Connections. Regional Employment Connections includes information about local job leads, opportunity to network with other job seekers, and notice of upcoming events and job fairs in the community.

Provides programs that help individuals to remain in their homes and connected to the community. Services may include transportation to medical appointments, shopping, and friendly visits, community nurse visits, referrals for other services, computer technical support, wellness classes, and activities.

Provides post-permanency services including in-home adjusted parent education, consultation to treatment teams and advocacy, referrals to community resources, general adoption information, and kinship placements support.

Provides comprehensive medical care to individuals of all ages including physical exams, immunizations, and routine screenings, treatment of short-term injuries or illnesses, treatment of chronic illnesses, care for women during pregnancy and childbirth, family planning services, nutritional counseling, physical therapy, and other special programs. Provides preventive health care, as well as diagnosis and treatment of illness in children, adolescents and adults. Services include primary medical care such as physical health examinations, immunizations, acute and chronic health screenings, obstetric care, laboratory testing, and birth control counseling. Provides onsite integrated health services to enhance access to appropriate prevention, treatment, recovery and wellness services and activities. Pediatric primary care is staffed by specialists who are concerned with the physical, emotional and social health of infants, children and adolescents from birth to young adulthood.

Provides community food shelf services. Personal care items may be available depending on donations. Vouchers are available at the discretion of the provider for items not available at food shelf, and are redeemable at Wing's, the local grocery store, for food and personal items only.

Works to protect the public's health by effecting changes at the environmental, policy, and systems levels. The goals of the Tobacco Control Program are to prevent youth and adults from becoming tobacco users, to support those looking to quit, and to change the overall acceptance of tobacco use.

Offers a victim centered, pre-charge offender program that provides an opportunity to the offender to participate in an out-of-court process that, upon successful completion, will result in no legal charge being brought against them.

Offers low-interest loans of up to $20,000 for energy efficiency improvements and heating systems. Eligible projects include cold-climate heat pumps; central wood pellet heating systems; cord wood and pellet stoves; heat pump water heaters; solar hot water; and weatherization improvements. Health and safety measures and repairs needed for the efficiency improvement can constitute up to 50% of the total loan amount. Zero-percent interest financing is available for low to moderate-income approved applicants.

Provides youth employment services that include vocational testing and assessment to help identify skills, talents and experience; career guidance to help plan and map out a career strategy; tutoring and study skills training; summer employment opportunities; paid work experience including internships and job shadowing; leadership development opportunities; and support services to help overcome employment barriers.

Provides a full range of inpatient and outpatient care and 24-hour emergency care. Services include 24-hour clinician staffed emergency room services, diagnostic imaging, general medical and surgical services, lab testing, blood supply services, hospital-based outpatient services, disease specific treatment, pain management, adult and children's services.

Provides telephone and face-to-face crisis intervention services for individuals whose emotional distress may range in severity from difficulty in coping with daily stress to the crisis of a psychotic episode. Enhanced Mobile Crisis Services provide rapid mobile response for individuals experiencing a mental health or substance use disorder crisis. A trained 2-person team can meet the individual at home, workplace, or another community setting, providing support and comfort including safety plans, peer support, care coordination and follow up and, if needed, temporary overnight accommodation.
