Provides assistance with locating, securing and maintaining permanent, affordable housing and will provide intensive case management individually tailored to meet the needs of each family. Counselors work with families to assess needs and to establish housing goals. Provides information and guidance about rental costs, renter responsibilities, how to select affordable housing that meets individual needs, and how to budget for insurance, utility costs and other requirements related to paying for housing. Also provides tenant rights information and landlord/tenant mediation as part of individualized case management.

Provides a program that empowers Vermonters to lead healthier lives through hands-on nutrition education. The Expanded Food & Nutrition Education Program (EFNEP) is usually a six-session series held in home, school, or community settings. An online nutrition education series, Eat Smart, Move More, is also offered and participants can access lessons at their own pace while receiving one-on-one support from their closest EFNEP Educator via phone calls, zoom meetings, and emails. Topics covered may include meal planning and preparation, food budgeting with a limited income, food safety and preservation, and nutrition tips.

Partners with senior meal sites and senior centers to provide hot, nutritious, noontime meals. Home-delivered meals are often available for those who are unable to reach a meal site because of illness or frailty.

Represents individuals who receive long term care services in nursing homes, residential care homes, assisted living residences, or their own homes through the Choices for Care Program. Provides professional assistance with the assessment and development of plans for the care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for in-home care services. Putney's community Registered Nurse will coordinate the delivery of needed services, provide assistance/education around following medical advice and proper medication taking, assess in-home mobility safety to reduce the risk of falls, assist families with understanding the issues and needs of the family member requiring in-home care services, and provide ongoing follow-up visits to ensure the best possible health outcomes. All services provided are confidential and specific to each individual's needs.

Provides a telephone and/or face-to-face response to individuals in crisis. This service offers an immediate intervention and suicide prevention response by mental health counselors and connects individuals to resources that best fit the needs of their particular situations. Within the First Call program there are specialized services such as crisis case management, crisis respite, support services when a tragic event impacts a child, as well as Safety Net, an urgent response service for families impacted by incarceration. First Call also provides education, support, and "Gatekeeper Training" to community providers to promote prompt and appropriate response to mental health crises.
