Provides comprehensive services to individuals with developmental disabilities, autism, and other pervasive developmental disorders. Individuals are assigned professional service coordinators who are responsible for clients' Individualized Support Agreements, and for assisting them in achieving success in the community. Services include personal support and case management, specialized residential programs, psychiatric and medical care, vocational and skill acquisition training, family support services, and support services to individuals in nursing homes. Provides a spectrum of supported living arrangements to meet the needs of each person. Provides confidential, client-directed information and assistance in gaining access to government and community programs including housing, transportation, Meals on Wheels, congregate meals, food stamps, Medicaid, health-care insurance, legal services, adult day services, hoarding issues, and mental health services. Options Counseling is also available to enable older adults to maintain independence. Maintains a regional listing of in-home assistance providers available to assist seniors with continuing to live independently at home.

The Hartford community's registered nurse will coordinate the delivery of needed services, provide assistance/education around following medical advice and proper medication taking, assess in-home mobility safety to reduce the risk of falls, assist families with understanding the issues and needs of the family member requiring in-home care services, and provide ongoing follow-up visits to ensure the best possible health outcomes. All services provided are confidential and specific to each individual's need. Referral forms can be found on the website. Forms are only viewed by the nurse. Clients will be contacted directly within 1-2 weeks.

Host home providers are provided with a respite budget for periods of relief or rest from caregiving.

COMMUNITY CONNECTIONS | NORTHEASTERN VERMONT REGIONAL HOSPITAL

Provides holistic care services to all community members that may include the following: Nursing, Behavioral Health and Physical Health Coaching services: Nutrition education, exercise education and counseling, short-term behavioral counseling, Diabetes education, individual health coaching, medication management, weight management-individual and group support, smoking cessation, Advanced Directives, Fall Safety assessments, medical transportation and dental solutions. Community Connections Community Health Team also provides Resource Advocates who will assist with completing applications for the following services and assistance: health insurance (VT Health Connect and Medicare and Medicare D comparisons), SSDI/SSI, and prescription costs, as well as application assistance for housing, food, and utilities. The team also includes VT Health Connect navigators and assistors, as well as care managers for individuals who require support maintaining independence and stability within the community. Provides support services for patients and their families during hospitalization and upon discharge, for people receiving outpatient services, for previous patients and for other people not previously associated with the facility who need the service. Implements a Community Health Improvement Program based on the Community Needs Health Assessment.

CASEWORK ASSISTANCE | TRAGEDY ASSISTANCE PROGRAM FOR SURVIVORS

COMPASS | NEKCA COMMUNITY AND SOCIAL JUSTICE DEPARTMENT

Provides crisis and stabilization services to at risk youth and families. Assists youth with supports toward developing well-being, education, employment, permanent connections, and stable living environments as they transition to adulthood. Compass works with youth and their families to make a plan of care that will assist them in accomplishing their goals. Youth meet with a case manager approximately once a week. If a youth's family is available for services, they will have the opportunity to work with Compass' Family Preservation Specialist around normal adolescent development, verbal de-escalation techniques, mediation, and other approaches. Provides crisis management for enrolled clients and families 24 hours a day through a pager. There is a rotating staff member on call at all times.

Strafford's community Registered Nurse will coordinate the delivery of needed services, provide assistance/education around following medical advice and proper medication taking, assess in-home mobility safety to reduce the risk of falls, assist families with understanding the issues and needs of the family member requiring in-home care services, and provide ongoing follow-up visits to ensure the best possible health outcomes. All services provided are confidential and specific to each individual's needs. Office hours at United Church of Strafford (UCS), 230 Justin Morrill Highway, Strafford 05072 are available on request. The nurse does not respond to emergency calls (use 911) or provide "hands-on" nursing such as: diagnosing or treating illness, changing dressings, or administering IV's.
