VERMONT CHRONIC CARE INITIATIVE | DEPARTMENT OF VERMONT HEALTH ACCESS
Offers short term, holistic, intensive clinical case management, and social support services. Vermont Chronic Care Initiative (VCCI) licensed case managers meet with members in their homes to work on self-management plans. Case managers collaborate with providers and attend provider appointments to help facilitate collaborative care planning, meet clinical gaps in care. They also work with community/state agencies to address other needs, such as housing and transportation.

Eligibility
Medicaid recipients, whether or not they also have other insurance, who have one or more chronic conditions such as asthma, congestive heart failure, depression, diabetes, coronary artery disease, chronic obstructive pulmonary disease, low back pain, mental health and substance use/abuse disorders, and have multiple emergency room visits and inpatient admissions
