Issues Social Security numbers and Social Security cards, as well as replacements for lost or stolen cards for all United States citizens. A Social Security card/number identifies the monthly benefits for individuals who are fully insured. Provides information about and assists with filing for social security benefits. Benefit amounts depend upon date of birth, wages earned, and the type of benefit.

Provides a disaster assistance website and Disaster Assistance Helpline that allows visitors to get information about location of disasters, how to meet their immediate needs, as well as to apply for and check the status of assistance applications.

Provides home pre-purchasing counseling and foreclosure prevention counseling, reverse mortgage counseling, as well as general home buyer education.

Provides professional assistance with the assessment and development of plans for the care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for in-home care services. Chelsea's community Registered Nurse will coordinate the delivery of needed services, provide assistance/education around following medical advice and proper medication taking, assess in-home mobility safety to reduce the risk of falls, assist families with understanding the issues and needs of the family member requiring in-home care services, and provide ongoing follow-up visits to ensure the best possible health outcomes. All services provided are confidential and specific to each individual's needs.

Provides a community breakfast meal

Provides prenatal and postpartum support including childbirth education, breastfeeding support, parenting strategies, coping skills, and information on health and safety issues.

Offers a home-like atmosphere, including a full kitchen, living room with computer access, washer/dryer, shower, TV, and two "quiet rooms", for families supporting children in the hospital.

Provides health care services at all community outpatient clinics. Homebound Veterans are served by Home Based Primary Care (HBPC). Additional services can include basic nursing services, telemedicine, patient advocacy, hospice care, and physical, occupational, or speech therapies through a Skilled Home Care service.

Provides primary police services for approximately 200 Vermont towns out of 10 barracks throughout the state. Troopers perform traditional police services of traffic enforcement and the investigation of crimes, and also enforce snowmobile and recreational boating laws.

Provides monthly benefits to eligible survivors of workers covered under Social Security who have died. Survivor benefit amounts are a percentage of the deceased worker's primary insurance amount.

Provides comprehensive services to individuals with developmental disabilities, autism, and other pervasive developmental disorders. Individuals are assigned professional service coordinators who are responsible for clients' Individualized Support Agreements, and for assisting them in achieving success in the community. Services include personal support and case management; specialized residential programs; psychiatric and medical care; vocational and skill acquisition training; family support services; and support services to individuals in nursing homes. Provides a spectrum of supported living arrangements to meet the needs of each person. Clinical services for individuals include provision of a psychiatrist to provide evaluation, medication checks, and after-hours consultation on medication issues. Also provides care coordination services.

Offers a substance free environment for peer support, educational programs, information and referral to needed services, recovery coaching, socializing, recreational activities, and meeting space for a variety of recovery workshops and meetings. Also provides a community health vending machine which dispenses harm reduction and hygiene supplies located outside on the center's grounds.

Provides talking books, large-print books, Print/Braille picture books, magazines on tape and descriptive videos. Provides a toll-free, electronic service that allows individuals who are blind or disabled to listen to daily newspapers and magazines. Arranges for home delivery of books and audiovisual materials to individuals unable to leave their homes.

Provides formal supervision of offenders released conditionally on probation or parole or confined to their homes or other venues in the community.

Provides confidential 24-hour telephone support. Person-directed services may include safety planning, emotional support, assistance in obtaining emergency relief from abuse orders, hospital emergency room support, assistance in finding emergency shelter, therapeutic treatment planning assistance, advocacy for legal, financial, medical, and housing issues, and community resource referrals.

Represents individuals who receive long term care services in nursing homes, residential care homes, assisted living residences, or their own homes through the Choices for Care Program. Issues addressed include quality of care, quality of life, and residents' rights.

Provides community food shelf services. Personal care items may be available depending on donations.

Provides primary health care, as well as prevention, diagnosis, and treatment of illness. Services include primary medical care such as physical health examinations, immunizations, acute and chronic health screenings, obstetric care, laboratory testing, and birth control counseling. Provides onsite integrated health services to enhance access to appropriate prevention, treatment, recovery and wellness services and activities. Pediatric primary care is staffed by specialists who are concerned with the physical, emotional and social health of infants, children and adolescents from birth to young adulthood.

Provides safety information regarding smoke alarms, carbon monoxide alarms, and fireworks.

Provides Work Incentives Counselors who can help individuals decide whether to work, keep necessary benefits while they work, and avoid work/benefits conflicts. The program is also available to youth in high school who are in transition to adult life. Youth and their families can receive accurate information about the impact of employment on all Federal and State benefits they receive. Provides the Vermont Work/Benefits Helpline.
