Provides patient-centered physical and mental health care delivered by a team of primary care and mental health clinicians .The clinics also provides referrals to other health and non-health services, and will screen patients and callers for eligibility for other health services and public insurance programs, as well as assisting with the application process. The Brandon, Castleton, and Mettowee clinics also hold diabetes education classes. This location also serves as an Express Care urgent care clinic.

Co-sponsors an annual Bike Safety Fair with Northeastern Vermont Regional Hospital providing residents with information about safe biking practices and offering bike helmets to all participants. A minor bike repair station may also be available.

Provides comprehensive services to individuals with developmental disabilities, autism, and other pervasive developmental disorders. Individuals are assigned professional service coordinators who are responsible for clients' Individualized Support Agreements, and for assisting them in achieving success in the community. Services include personal support and case management; specialized residential programs; psychiatric and medical care; vocational and skill acquisition training; family support services; and support services to individuals in nursing homes. Provides a spectrum of supported living arrangements to meet the needs of each person. Clinical services for individuals include provision of a psychiatrist to provide evaluation, medication checks, and after-hours consultation on medication issues. Also provides care coordination services.
