Assessments are provided both in home or pre-dismissal from hospitals or rehabilitation centers to determine eligibility for a variety of programs including Medicaid, SSI, food stamps, fuel assistance, housing, transportation, state pharmacy assistance programs, Lifeline telephone credit, respite and in-home services. Provides assistance with filling out forms for submission. Case managers also work with the VA in assisting veterans with applying for the Veterans Independence Program.

The Sharon Health Initiative's community health care worker will coordinate the delivery of needed services like adequate food and shelter and overall management of th individual's health well-being. This may include provision of assistance/education around following medical advice and proper medication taking, assessing in-home mobility safety to reduce the risk of falls, assisting families with understanding the issues and needs of the family member requiring in-home care services, and providing ongoing follow-up visits to ensure the best possible health outcomes. All services provided are confidential and specific to each individual's needs.

SARCs and Advocates will explain military reporting options available to the victim and connect them to appropriate medical, behavioral health, legal, spiritual, and financial resources available through the military.

The Victim Witness Specialist also maintains and distributes the Vermont Victim Services Resource Directory, a comprehensive guide to Vermont's state and federal resources that is made available to all service providers in Vermont.

Provides assistance for people who need housing including information and guidance about rental costs; how to select affordable housing that meets individual needs; and how to provide for insurance, utility costs, and other requirements related to paying for housing. Coordinates with other providers in the community to help individuals and families obtain or maintain housing. May offer limited financial assistance with rent, rental deposit, and other payments. Also provides the Rent Right Program, designed to provide participants with the skills, materials, and confidence necessary to find and maintain stable housing. Upon completion of the program, participants receive a Preferred Renter Certificate, which shows landlords that they have made an investment into being a good tenant.
Provides legal advice or representation to individuals regardless of having filed a police report. Program assists survivors of domestic violence who want protective orders, or advice on divorce, custody, and visitation; elders and vulnerable adults facing abuse or financial exploitation; victims with housing, employment, or government benefits issues; and victims who need help safeguarding their rights through criminal proceedings.

Provides low-cost or free health care for adults. Vermont Medicaid includes doctor visits, hospital care, prescriptions, vision, and some dental care. Also covers long-term care in a nursing home or at home, and physical therapy. Medicaid for the Aged, Blind, or Disabled (MABD) is also available. Assists Vermonters in enrolling in (and answers questions about) Medicaid and MABD. The member services line can help with filling out the application.

Provides referrals for free, confidential mental health services of an hour per week for a year. Give an Hour also supplies a variety of links to mental health information and resources for military families. Works to reduce the stigma associated with mental health problems through education, training, and outreach efforts. Also provides free webinars for Wellness Ambassadors for promoting and advocating for mental health awareness in communities.
