Provides clients with social workers who offer coordination of community services and resources, client and family counseling, and navigating the health care system. Licensed Nursing Assistants provide home health assistance, including personal care assistance, light housekeeping, and help with normal daily living activities.

Provides health and welfare emergency communications during disasters or significant failures of telecommunication systems through a volunteer organization of amateur radio operators who are licensed by the Federal Communications Commission (FCC). Radio Amateur Civil Emergency Services (RACES) volunteers can be activated to provide alternative communications channels following a major disaster or large-scale emergency that disrupts the normal functioning of a community. There are currently 40 fixed RACES stations with the capability of deploying 15 mobile Command Centers stations throughout the State.

Supports successful community reentry for individuals released from incarceration through Circles of Support and Accountability (COSAs). Circles consist of highly skilled staff and trained community volunteers; helping individuals navigate daily challenges and take responsibility for ones actions while building a path forward through skill building, proper disclosure, conflict resolution and healthy pro-social skills. May also assist in connecting participants with employment, housing, transportation, and benefit programs.

Provides a full range of supportive services for individuals with a limited life expectancy and their families. Also provides comprehensive palliative and support care to individuals in need of pain and other symptom management, whether or not they have a life limiting illness.

Renovates and repairs homes in partnership with eligible families. Projects may include essential minor repairs, accessibility projects, painting, landscaping, and basic weatherization measures to make homes safer and more energy efficient.

Provides physical health exams, acute and chronic health care, immunizations, women's health screenings, limited mental health counseling by appointment, lab and blood work, and assistance with finding affordable prescription medications. Provides patient-centered physical and mental health care delivered by a team of primary care and mental health clinicians .The clinics also provides referrals to other health and non-health services, and will screen patients and callers for eligibility for other health services and public insurance programs, as well as assisting with the application process. Also hold diabetes education classes.

Provides loans and/or grants to municipalities and nonprofit organizations to purchase or expand community facilities and equipment, including public services like hospitals, libraries, fire/police stations and schools, water and waste management, and local food systems. Also provides disaster relief funding for communities and organizations impacted by catastrophes.

Hears appeals of decisions by the Agency of Natural Resources including state land use permit decisions and state environmental permits.
