Provides a bi-weekly community lunch and dinner. Local businesses or organizations may volunteer to organize, set up, break down, and serve the meal. Reimbursement by Grafton Cares for the cost of ingredients is possible if desired.

Provides the elderly and frail with daily check-in calls from the dispatcher. If there is no answer and recipient has not previously indicated being away, a listed first responder or law enforcement officer will be dispatched to check.

Administers programs that work with farmers and others in the food system to engage in and share research, information, technical assistance, and educational opportunities. The Pasture Program offers technical and outreach assistance to help livestock farms reduce purchased feed and labor costs to maximize local farmland and assets. The Land Access Program helps new and existing farmers find the land they need to grow crops, raise livestock, and care for the natural environment while building financial viability. The Good Agricultural Practices (GAPs)/ Farm Produce Safety program helps farmers learn the best ways to ensure the safety of their produce, an increasingly common requirement among retail and institutional buyers. Our Local Food program fosters relationships between farms and food-insecure people, gives farmers resources to help them work effectively with food access organizations, and helps identify policy actions to support getting Vermont-produced food to under-served Vermonters.

Provides comprehensive assistance for people who want to purchase housing, including information and guidance about buying costs; how to select affordable housing that meets individual needs; and how to provide for insurance, maintenance and other requirements related to acquiring and paying for housing.

Provides community food shelf services. Personal care items may be available depending on donations.

Provides information about child care providers, such as when they became licensed, if they are accredited or have a STARS rating, and if they've had any licensing violations in the past year. Also provides a way to voice a concern about a provider (for example, they are unregulated, there is lack of supervision or too many children, unqualified or not enough staff, inappropriate guidance and discipline, unsafe environment), or make a formal complaint about a child care provider.

Provides low-cost or free health care for adults. Vermont Medicaid includes doctor visits, hospital care, prescriptions, vision, and some dental care. Also covers long-term care in a nursing home or at home, and physical therapy. Medicaid for the Aged, Blind, or Disabled (MABD) is also available. Assists Vermonters in enrolling in (and answers questions about) Medicaid and MABD. The member services line can help with filling out the application.

Offers apprenticeships in the following areas: Certified Public Bookkeeper, Manufacturing Production Technician, Medical Assisting, Medical Records Assistant, and Pharmacy Technician. All offerings pair credit-bearing classes with on-the-job learning.

Provides a webpage with a map of Vermont divided into towns. Each town is color-coded to indicate which Solid Waste Management District (SWMD) covers that town. The key to the map provides the phone numbers for all the SWMDs. The website itself (as well as individual SWD websites) contains important information about hazardous waste disposal, including how to properly dispose of such items as computers, tires, and batteries. The website also provides detailed information about Act 148, Vermont's Universal Recycling Law, which covers how to comply with the law's requirements on how to dispose of plastic, paper, yard debris, clean wood, food scraps, and other organic materials.

Provides comprehensive outpatient substance use disorder treatment services that include substance use disorder counseling, medication-assisted treatment, integrated medical and behavioral services, treatment and counseling for individuals with co-occurring disorders, and psychiatric medication management.

Provides preventive health care, as well as diagnosis and treatment of illness in children, adolescents and adults. Services include primary medical care such as physical health examinations, immunizations, acute and chronic health screenings, laboratory testing, and birth control counseling. Provides onsite coordinated primary care, mental health services, and substance abuse services to enhance access to appropriate prevention, treatment, recovery and wellness services and activities. Programs for health maintenance and public health awareness services are also offered. Coordinates and supports provision of non-emergency medical transportation through local transportation programs to health and wellness appointments.

Co-sponsors an annual Bike Safety Fair with Northeastern Vermont Regional Hospital providing residents with information about safe biking practices and offering bike helmets to all participants. A minor bike repair station may also be available.

Provides an online directory of libraries that includes public/private town libraries and school libraries with addresses, contact information and hours.

Provides support to children and teens through prevention education, individual advocacy, and group support. Provides confidential advocacy and support services for teens around Teen Dating Violence, Sexual Violence, Stalking, Healthy Relationships, and Consent.

Offers one-on-one and group HIV prevention education and support, prevention supplies and materials, peer outreach program, and HIV-positive speakers bureau.

Provides MAT (medication-assisted treatment) for adults struggling with opioid use disorder. Provides assessment and treatment for opiate addiction. Services include screening intake, assessment, treatment planning, methadone maintenance and medically supervised withdrawal, counseling services, case management and referral, medical services for opioid disorder, discharge planning, and follow-up.

Helps to connect emergency management and the general public with open pharmacies within an affected area during a disaster.

Provides occasional nursing and rehabilitative therapies in the patient's home. Skilled Nursing Services include nursing care, case management, and client and family education. Provides clients with social workers who offer coordination of community services and resources, client and family counseling, and navigating the health care system. Licensed Nursing Assistants provide home health assistance, including personal care assistance, light housekeeping, and help with normal daily living activities.
