KIDS-A-PART | LUND

Offers a variety of services aimed at keeping families connected when a parent is incarcerated, including case management and coordination of family care plans. The program also facilitates opportunities for incarcerated parents to communicate with their children, including individual and group visits in the child-friendly Kids-A-Part visiting space. Case management for families/caregivers in the community include financial support to facilitate visitation. Services also include statewide education and advocacy to community members and groups working with children.

DEVELOPMENTAL SERVICES PROGRAM | HCRS

Provides comprehensive services to individuals with developmental disabilities, autism, and other pervasive developmental disorders. Individuals are assigned professional service coordinators who are responsible for clients' Individualized Support Agreements and for assisting them in achieving success in the community. Services include personal support and case management; specialized residential programs; psychiatric and medical care; vocational and skill acquisition training; family support services; and support services to individuals in nursing homes. Provides a spectrum of supported living arrangements to meet the needs of each person. Clinical services for individuals include provision of a psychiatrist to provide evaluation, medication checks, and after-hours consultation on medication issues. Also provides care coordination services. Flexible Family Funding is a secondary service that provides state funds to support individuals with developmental disabilities and their families with support services such as respite care to enhance their ability to live together.

TRANSITIONAL LIVING PROGRAM | ELEVATE YOUTH SERVICES

Provides long term housing, life skills training, emotional support, as well as assistance in finding area services such as job training, counseling, education, child care, and health care after an initial assessment stage that determines appropriateness for participation in the program. Hotline Counselors are On Call 24Hours/7Days. Transitional Living Program also offers limited support with transportation, personal needs, and support in acquiring ID's such as birth certificates and social security cards.
