Provides comprehensive medical care to individuals of all ages including physical exams, immunizations, and routine screenings, treatment of short-term injuries or illnesses, treatment of chronic illnesses, care for women during pregnancy and childbirth, family planning services, nutritional counseling, physical therapy, and other special programs. Provides preventive health care, as well as diagnosis and treatment of illness in children, adolescents and adults. Services include primary medical care such as physical health examinations, immunizations, acute and chronic health screenings, obstetric care, laboratory testing, and birth control counseling. Provides onsite integrated health services to enhance access to appropriate prevention, treatment, recovery and wellness services and activities. Pediatric primary care is staffed by specialists who are concerned with the physical, emotional and social health of infants, children and adolescents from birth to young adulthood.

Provides short-term assistance such as security deposit, first month's rent, rent subsidy (up to three months), or back rent to prevent eviction.

Topics include movement and exercise, nutrition, childbirth education, car seat safety, chronic disease self-management, and health/disease specific support group meetings.

Provides nurses and/or other trained professionals who visit the homes of women during pregnancy and after childbirth to help improve their personal health and development during the prenatal period and learn to care for their infant. Services include at-home assessments for expectant mothers and infants, postpartum assessments, newborn evaluations, breastfeeding support, nutrition education, parenting information, family adjustment support, skilled nursing care if needed, and assistance with coordination of family services.

Partners with senior meal sites, senior centers, and restaurants to provide hot, nutritious, noontime meals. Home-delivered meals are often available for those who are unable to reach a meal site because of illness or frailty.

Provides a monthly community lunch to eat in or take out.

Provides information and assistance with Medicare, including Part D Drug benefit, Medigap Supplemental Insurance; managed care and long term care insurance; durable medical equipment; and pharmacy assistance programs. Healthcare Benefits Specialists can explain how the different programs work, can help sort through medical forms and bills, and can evaluate medical insurance needs.

Provides community food shelf services. Personal care items may be available depending on donations. Also provides a voucher for a Thanksgiving basket.

Provides comprehensive outpatient substance use disorder treatment services that include substance use disorder counseling, medication-assisted treatment, integrated medical and behavioral services, treatment and counseling for individuals with co-occurring disorders, and psychiatric medication management.

Provides flexibly scheduled and routed door-to-door (or curb-to-curb) transportation services. Coordinates and provides non-emergency medical transportation to health and wellness appointments, including for individuals who do not qualify for Medicaid or Senior Rides transportation programs. Offers transportation for individuals with mental health and/or substance use disorder to support groups, job training and access, recovery centers, drug testing, and other services. Provides transportation service to and from areas outside of the Northeast Kingdom and the state, provided the client prepays the negotiated cost of the trip(s). Provides individual and group therapy to individuals who have been negatively impacted by acute and/or chronic exposure to traumatic events at work. Common reasons to seek treatment include: changes in mood, constant vigilance, difficulty with personal relationships, disciplinary actions at work, suicidal thoughts, or using alcohol or drugs to help manage difficult feelings.

Provides family dentistry services, including comprehensive clinical care and prevention services. Comprehensive clinical care services include: preventive, restorative, extractions, crown and bridge, periodontal services and minor implant restorations.

Offers low barrier buprenorphine program with same day access to life saving medication. Provides HIV, Hepatitis A/B, and Hepatitis C testing and vaccinations, access to safer injection supplies, safe disposal of used supplies, fentanyl test strips, xylazine test strips, drug treatment options, and counseling. Distributes overdose rescue kits that contain naloxone to protect people from the potentially deadly effects of the drugs they use. Offers a Mobile Needle Exchange van with other services in Chittenden and Franklin Counties. Also provides needle pick-up service and needle handling safety information. Offers AIDS/HIV/STD prevention supplies.

Offers weekend programs for those who would like to explore science, ecology, the environment, and environmental justice while expanding their leadership skills. Participants will have an opportunity to explore nature, learn about the environment, and be hands on with outdoor projects as a way to give back to the community.

Offers a way to compensate for food lost during a disaster, such as a flood or fire, loss of electricity, or equipment failure. Recipients are eligible to have a portion or all of their benefit replaced, depending on the value of the food they lost due to the disaster.

Recruits volunteers and places them in community service jobs at non-profit organizations based on their experience, knowledge, and skills. Volunteers receive training, assessment, recognition for their contributions, and supplemental insurance coverage.

Provides an online directory of community resources and services to assist families of children with special needs. Offers affordable educational and recreational programs that are specifically tailored to children with special needs, including skating, soccer, adaptive running, dancing, and art classes. Purchases adaptive equipment for local organizations.

Provides information, resources, and support to individuals who have experienced crime and harm , even if the individual responsible is never identified or apprehended. Types of assistance include emotional support, safety planning, referrals to other organizations, systems advocacy, and limited financial assistance.

Meets people with empathy and practical tools, offering one-on-one problem-solving to meet urgent needs while helping to promote long-term stability. Prevents utility shutoffs and restores heat and electricity, Stabilizes housing by providing emergency rental and back rent assistance, Supports flood recovery and disaster response, Helps unhoused families and individuals move into permanent housing. Also offers Community Resource and Food navigation and Application support.

Administers the registration and title of all motor vehicles. Issues Vermont license plates. Issues Vermont Driver Licenses, Commercial Driver Licenses, School Bus Driver Licenses, and Motorcycle Driver Licenses. Provides driver training information and issues ID (Non-driver) cards and Learner Permits. Provides school bus driver training information and administers scheduling for driver's license exams. Issues Foreign and International Licenses.

Provides supplemental food for families with young children. WIC also provides breastfeeding support and nutrition education. Participants can get Farm to Family coupons to use at local farmers' markets.

Provides a program that empowers Vermonters to lead healthier lives through hands-on nutrition education. The Expanded Food & Nutrition Education Program (EFNEP) is usually a six-session series held in home, school, or community settings. An online nutrition education series, Eat Smart, Move More, is also offered and participants can access lessons at their own pace while receiving one-on-one support from their closest EFNEP Educator via phone calls, zoom meetings, and emails. Topics covered may include meal planning and preparation, food budgeting with a limited income, food safety and preservation, and nutrition tips.
