Offers innovative and often very intensive assistance to youth transitioning to adulthood with accessing and succeeding in employment, education, housing, positive relationships, community connections, and living independently. The JOBS program focuses on youth who have either left or are seriously at-risk-of leaving school prior to successful graduation and are at high risk for involvement with the criminal justice system, substance use, homelessness, physical abuse, abusive behaviors, or other areas of significant concern.

Provides advocates who are deaf in support of the domestic violence hotline services for individuals who are deaf, deaf/blind, or hard of hearing by videophone, instant message, live chat, or email. Services include crisis intervention, safety planning, and information and referrals to local agencies.

Also provides a registered nurse for home health services. Services may include assessments, consultation, case management, foot care, uncomplicated wound care, and blood pressure screenings. Services available to participants may include transportation, companionship, social activities, opportunities for community involvement, and help with home chores.

Provides comprehensive lead poisoning prevention information, resources, and technical assistance to parents and caregivers.

Offers short-term residential services to support a young person's transition from homelessness to independence. Services include assessment of independent living skills, case management, transitional living support, and outreach services. Life skills training, health and parenting classes are also offered.

Provides support, and assistance to people going through a life-altering illness, one that may or may not have a terminal prognosis. Offers one-on-one volunteer services, support groups with peers, and opportunities to learn more about what resources are available in the community.

Provides a shelter program including case management. Also provides transitional housing for some qualifying shelter participants as a next step toward stable housing.

Offers a diversion-like program that connects participants with substance use, mental health, and other community-based service supports, regardless of the participants criminal history, with the aim of improving the person's health and reducing future adverse involvement in the justice system. Successful completion of program results in a dismissal of the criminal charge.

Provides financial assistance of up to 50,000 dollars per unit to make vacant rental units available by assisting with health and safety repairs, as well as repairing code violations such as electrical, plumbing, water, septic, heating systems, roof, accessibility, and other structural repairs. Program participants are required to provide a 20% match of grant funds and rent units at or below HUD Fair Market Rent prices. Program can also assist with the creation of new units, including accessory dwelling units.

Provides care to LGBTQ2+ Veterans. Services include mental health care, hormone therapy, HIV prevention, testing, and other STI treatments and screenings. Also provides gender affirming prosthetics, transgender and other LGBTQ2+ support groups and services.

Offers a virtual social time.

Provides physical fitness activities to individuals in recovery from substance use disorder, such as crossfit, yoga, and strength training classes. Also provides a variety of health and wellness online classes and events. There is also a mobile app that lists classes and events in the community.

This group is focused on the range of emotions that can arise when expecting after experiencing infertility and/or loss.

Facilitates support groups by trained support coordinators for new parents/caregivers who are at risk of or are experiencing distress such as anxiety, depression, trauma, and other difficulties. Facilitates social circles to bring new parents/caregivers together to foster a sense of community so they feel connected and less alone in their unique-but-shared experiences.

Provides shelter services for families with minor children experiencing homelessness.

Conducts confidential ongoing outreach to current and former military members and their families, to identify potential needs and provide access to available services, including advanced outpatient mental health clinical services such as counseling and traumatic brain injury assessment. Also provides dental referral for veterans. Members of the outreach team can meet with veterans and families at home, at work, or any place in the community.

Helps eligible parents earn two- or four-year undergraduate college degrees with the goal of getting good paying jobs so they can support their families. The program provides monthly cash payments, support services, help with childcare, transportation, and school supplies, and may provide benefits through Reach Ahead if the parent is working and the PSE grant closes.

Assists youth in developing the life and living skills necessary to aid in the transition to independent living and adulthood. Educational resources include budgeting, employment, and application assistance. Case management and support s provided to youth and young adults who receive tenant based rental assistant with vouchers and administered monthly rental payments. Mediation is offered to support clients with communication with landlords, mitigating issues and concerns which arise to ensure secure housing for the youth and young adults.

Provides low-cost or free health insurance for children and pregnant people. Benefits include doctor visits, emergency services, prescription medicines, dental care, hospital visits, vision care, mental health care, substance use disorder treatment, immunizations, and services for pregnant women such as lab work and tests and prenatal vitamins.

Provides professional support partners to connect individuals to MS information, resources, and support. Helps to navigate the challenges of MS unique to each situation, providing a range of services: Information and education to help advocate for what is needed; Emotional support resources; Help with the complexities of finding a healthcare provider, benefits, insurance and access to coverage, and employment; Resources to face financial challenges and plan for the future; Wellness strategies including diet, exercise, emotional well-being and connection to local wellness resources; and Assessment for personalized case management in times of need.

Provides a searchable listing of camps in Vermont for children with disabilities.

Provides education and technical assistance geared to the needs of women farmers at all stages of their careers. Through a series of educational, technical assistance, and networking opportunities, works to increase the number of women owning and operating profitable farms and ag-related businesses, as well as their profile in leadership positions throughout the agricultural sectors of business, government, and community.

Coordinates monetary donation and funding response that supports Vermont farmers recovering from the flooding across the state in 2024.

Provides a wide variety of virtual groups for support and socialization for people living with epilepsy and/or their parents/caregivers. Trained coordinators match individuals with the appropriate group.
