Provides comprehensive, coordinated long-term care in an individual's home. Services include case/care management, homemaker assistance and assistance with the activities of daily living, including personal care and respite care that enables frail elderly and permanently disabled individuals to live safely, comfortably and as independently as possible at home. Also provides Medicaid Waiver, community-based one-to-one long term care services to individuals with Traumatic Brain Injury.

Maintains a wish list of items that individuals are looking for and assists with coordination of pick-ups as the items become available.

Supports the successful re-entry into the community of people released from incarceration through Circles of Support and Accountability (COSAs). COSAs are made up of citizen volunteers who work with participants to help them learn to manage the challenges of everyday life, be accountable for their actions and become responsible community members. Volunteer coordinators may help establish links to employment, housing, transportation, and benefit programs.

Provides an interactive decision-support process that helps individuals in need of long term care and their families understand the services and assistance available in the community. Helps the individual identify their strengths, needs, preferences, and unique circumstances to weigh the pros and cons of available alternatives. Includes a discussion of the factors to consider when making long term care decisions, and information about the range of long term care support options (such as personal care, transportation, and medication management). Also helps to identify resources that can help to pay for services. Maintains a regional listing of in-home assistance providers available to assist seniors with continuing to live independently at home.

Provides formal supervision of offenders released conditionally on probation or parole or confined to their homes or other venues in the community.

Provides in-home pediatric care under physician orders for intermittent skilled care and specific procedures. Provides physical therapists at request of physicians to do growth and development assessments, to establish exercise routines, to monitor progress, and to assist families with obtaining adaptive equipment.

Constructs energy efficient, affordable homes in partnership with eligible families. The houses are sold to the partner families at cost, with no profit and no interest. Costs are kept low by using donated materials and volunteer labor.

The Expanded Food & Nutrition Education Program (EFNEP) is usually a six-session series held in home, school, or community settings. An online nutrition education series, Eat Smart, Move More, is also offered and participants can access lessons at their own pace while receiving one-on-one support from their closest EFNEP Educator via phone calls, zoom meetings, and emails. Topics covered may include meal planning and preparation, food budgeting with a limited income, food safety and preservation, and nutrition tips.

Services include at-home assessments for expectant mothers and infants, postpartum assessments, newborn evaluations, breastfeeding support, nutrition education, parenting information, family adjustment support, skilled nursing care if needed, and assistance with coordination of family services.

Provides an offender re-entry program that is designed to address and repair the harm done to the community and the offender's victims while reducing the barriers to successful re-entry of offenders back into the community as citizens.
