The plan might include, weekly phone check ins, postpartum mood disorder screening, food and diaper distribution, and help coordinating additional resources as needed. Enrolled HFFTS program: Consists of a 3-week prenatal childbirth and parenting class followed by a weekly postpartum home visit up to twelve weeks postpartum (which can be done in person or remote), ongoing phone support, food/diaper distribution if needed, weekly circle of support held over zoom focusing on self care, newborn care and parenting, and help coordinating additional resources as needed. At the end of the twelve weeks, families can transition to additional ongoing Family Room programs and support.
