VERMONT MEDICAID | DEPARTMENT OF VERMONT HEALTH ACCESS
Provides low-cost or free health care for adults. Vermont Medicaid includes doctor visits, hospital care, prescriptions, vision, and some dental care. Also covers long-term care in a nursing home or at home, and physical therapy. Medicaid for the Aged, Blind, or Disabled (MABD) is also available. Assists Vermonters in enrolling in (and answers questions about) Medicaid and MABD. The member services line can help with filling out the application.

INSURANCE CASE MANAGEMENT AND REFERRALS | VALLEY HEALTH CONNECTIONS
Provides assistance in navigating Vermont Health Connect, Green Mountain Care and FQHC health care systems to aid clients in obtaining free or subsidized health care through referrals, including: benefit screening and enrollment assistance, dental referrals and assistance in obtaining care, patient assistance and pharmacy programs, eye care referrals, tobacco use screening and cessation program referrals, and physician referrals. All referred services providing direct care are based on income eligibility.
Outreach Referral Assistance appointments are available; drive-up appointments for patients with concerns or mobility issues are also available.

Provides community food shelf services. Personal care items may be available depending on donations. Provides access to USDA surplus commodity foods. May distribute Thanksgiving turkeys. All participants seeking food services are screened for possible USDA food assistance programs and 3SquaresVT eligibility. Delivery available to eligible individuals. No individual in need is denied services. Donations are accepted during food shelf hours. Monetary donations to the Food Shelf can be made in person or online.

Assists with attempts to access local and state emergency financial assistance when applicable. Personal budgeting counseling sessions are offered and made available to all individuals seeking financial assistance. Does not provide directly any financial assistance. Does provide guidance and support coordination to assist individuals in accessing other local and state financial assistance programs. Partner agencies with designated office hours and personnel are co-located at MFCC and include Vermont Adult Learning and Howard Center.

Grace Cottage's Community Health Team also provides a Resource Advocate who will assist with completing applications for the following services and assistance: health insurance (VT Health Connect and Medicare and Medicare D comparisons), SSDI/SSI, and prescription costs, as well as application assistance for housing, food, and utilities. Community Health Teams supplement the services available through primary care providers and serve to link individuals with the social and economic services that make healthy living possible for all community members. Each of Vermont’s Health Service Areas designs a Community Health Team to meet the needs of the local population and services may vary.
