Provides Medicaid Waiver, community-based one-to-one long term care services to individuals with traumatic brain injury. Services are offered to brain injured survivors following their post-acute care.

Provides intensive inpatient and outpatient medical services to treat specific traumatic injuries generally incurred in or around combat. Amputee rehabilitation, brain trauma, case care management, and physical therapy are all included in the medical services.

Helps Vermonters with moderate to severe traumatic brain injuries find a community-based living situation among family and friends. The program provides rehabilitation and support to individuals to help them maximize their independence and ultimately return to work. Services include rehabilitation, 24-hour care and supervision, crisis support, intensive counseling, medication review and instruction, employment supports to assist a person in finding and keeping a job, and other individualized supports and services. This program is short term in nature.
