Description
Helps Vermonters with moderate to severe traumatic brain injuries find a community-based living situation among family and friends. The program provides rehabilitation and support to individuals to help them maximize their independence and ultimately return to work. Services include rehabilitation, 24-hour care and supervision, crisis support, intensive counseling, medication review and instruction, employment supports to assist a person in finding and keeping a job, and other individualized supports and services. This program is short term in nature. Individuals requiring continued intensive one-to-one community supports may qualify for ongoing long-term services; A list of traumatic brain injury service providers can be found on the website
