Farm to Family Coupon Application and Distribution (first come, first served): is on the first week day in July. Coupons can only be redeemed at staffed farmstands, coupons can not be left at unstaffed farmstands that utilize a drop box.

Provides a program that empowers Vermonters to lead healthier lives through hands-on nutrition education. The Expanded Food & Nutrition Education Program (EFNEP) is usually a six-session series held in home, school, or community settings. An online nutrition education series, Eat Smart, Move More, is also offered and participants can access lessons at their own pace while receiving one-on-one support from their closest EFNEP Educator via phone calls, zoom meetings, and emails. Topics covered may include meal planning and preparation, food budgeting with a limited income, food safety and preservation, and nutrition tips. Offers phone, 802 QUITS, and in-person coaching, online support, access to free nicotine replacement (gum, patches and lozenges) and self-help tools, as well as to a Vermont Quit Partners Fresh Start Program series of weekly support meetings. Project Independence Caregiver Support Group can provide respite care during meetings if it's necessary to bring a loved one. Contact Project Independence the week before to request.

COMMUNITY HEALTH IMPROVEMENT | GRACE COTTAGE HOSPITAL

Provides holistic care services to all community members that may include the following Nursing, Behavioral Health and Physical Health Coaching services: Nutrition education, exercise education and counseling, short-term behavioral counseling, Diabetes education, individual health coaching, medication management, weight management-individual and group support, senior social isolation groups, Senior/ Disabled medical wellness home visits, smoking cessation, Advanced Directives, Fall Safety assessments, medical transportation and dental solutions. Grace Cottage's Community Health Team also provides a Resource Advocate who will assist with completing applications for the following services and assistance: health insurance (VT Health Connect and Medicare and Medicare D comparisons), SSDI/SSI, and prescription costs, as well as application assistance for housing, food, and utilities. Community Health Teams supplement the services available through primary care providers and serve to link individuals with the social and economic services that make healthy living possible for all community members. Each of Vermont’s Health Service Areas designs a Community Health Team to meet the needs of the local population and services may vary.
