ROAD TO RECOVERY | AMERICAN CANCER SOCIETY - NORTHEAST REGION

Provides round-trip transportation for individuals with cancer to scheduled treatment appointments. Volunteer drivers must complete an initial new volunteer training and subsequent annual training updates to ensure high-quality service delivery. A 5-7 Days Advance Notice Required. Rides are provided by volunteers and cannot always fill the ride requests.

Provides transportation to medical appointments and pharmacy visits. Provides flexibly scheduled and routed door-to-door (or curb-to-curb) transportation services. Coordinates and provides non-emergency medical transportation to health and wellness appointments, including for individuals who do not qualify for Medicaid or Senior Rides transportation programs. Offers transportation for individuals with mental health and/or substance use disorder to support groups, job training and access, recovery centers, drug testing, and other services. Provides transportation service to and from areas outside of the Northeast Kingdom and the state, provided the client prepays the negotiated cost of the trip(s). Offers volunteer driver opportunities.

COMMUNITY CONNECTIONS | NORTHEASTERN VERMONT REGIONAL HOSPITAL

Provides holistic care services to all community members that may include the following: Nursing, Behavioral Health and Physical Health Coaching services: Nutrition education, exercise education and counseling, short-term behavioral counseling, Diabetes education, individual health coaching, medication management, weight management-individual and group support, smoking cessation, Advanced Directives, Fall Safety assessments, medical transportation and dental solutions. Community Connections Community Health Team also provides Resource Advocates who will assist with completing applications for the following services and assistance: health insurance (VT Health Connect and Medicare and Medicare D comparisons), SSDI/SSI, and prescription costs, as well as application assistance for housing, food, and utilities. The team also includes VT Health Connect navigators and assistors, as well as care managers for individuals who require support maintaining independence and stability within the community. Provides support services for patients and their families during hospitalization and upon discharge, for people receiving outpatient services, for previous patients and for other people not previously associated with the facility who need the service. Implements a Community Health Improvement Program based on the Community Needs Health Assessment.

ADA / PARATRANSIT | GREEN MOUNTAIN TRANSIT

Provides transportation services for people with disabilities who are unable to use standard bus service. Service is provided within three-quarters of a mile of the fixed routes. Completed applications are evaluated within 21 days of receipt. Upon approval, individual can then contact Special Services Transportation Agency (SSTA) to make travel arrangements. Schedule by 4:30pm the day before travel.

TILLEY DRIVE SHUTTLE | SPECIAL SERVICES TRANSPORTATION AGENCY

Provides rides to a transportation hub, where riders can travel for any reason, including for health and wellness appointments. Provides a public transportation connection between the Downtown Transit Center (Cherry Street platform) and the offices at Tilley Drive off Hinesburg Road. Riders must provide location at Tilley Drive. Individuals traveling with a child up to age eight or weighing less than forty pounds must provide a child safety seat to ride in the shuttle.

NEIGHBOR TO NEIGHBOR | INTERFAITH COUNCIL OF THE NORTHSHIRE

Provides services to help people remain independent in their own homes. BARNARD HELPING HANDS | BARNARD HELPING HANDS

Provides a variety of services to support the health and well-being of neighbors on a temporary basis. Services may include transportation, friendly visiting, certificates and forms assistance, yard work, assistance with errands, homemaker assistance, and minor home repairs and maintenance. Also provides one-time emergency financial assistance to individuals facing a short-term crisis as well as referrals to other agencies for more long term needs. Barnard Helping Hands in partnership with Barnard General Store maintains a food collection box to support Woodstock Community Food Shelf. They also work closely with the Hub in Woodstock, the Ottauquechee Health Foundation and the Woodstock Thomspon Senior Center.

Recruits volunteers and matches interests and abilities with community members in need. Administers a volunteer program and coordinates local volunteers with senior community members who would like friendly visiting sessions, telephone reassurance calls, and minor home repairs. Provides volunteer drivers to assist with transportation to needed services such as medical, dental, grocery shopping, senior meal sites, court dates, housing appointments, etc. This service is not a formal transportation service and provision of rides is dependent upon volunteer driver availability.
