Provides rapid access to community-based housing. Services include housing search and placement, temporary financial assistance with rent and/or access to permanent housing subsidies, and intensive support services geared to the needs of individual households. Permanent supportive housing services include mental health and substance use supports from a multi-disciplinary team. Provides families and individuals with a transition program that moves them from experiencing homelessness to long-term self-sufficiency and housing stability. Program includes extensive case-management for budgeting, parenting, crisis management and intervention. Accepts referrals from case managers with a Release of Information (ROI). Housing Opportunity Program (HOP) funds are also available.

Provides intensive case management individually tailored to meet the needs of each family. Counselors work with families to assess needs and establish housing goals. Also provides assistance with locating, securing or maintaining housing.
