Provides physical health exams, acute and chronic health care, immunizations, women's health screenings, limited mental health counseling by appointment, lab and blood work, x-rays (Castleton and Mettowee only), and assistance with finding affordable prescription medications. Provides patient-centered physical and mental health care delivered by a team of primary care and mental health clinicians .The clinics also provides referrals to other health and non-health services, and will screen patients and callers for eligibility for other health services and public insurance programs, as well as assisting with the application process. The Brandon, Castleton, and Mettowee clinics also hold diabetes education classes. This location also serves as an Express Care urgent care clinic.

Offers Vermont high school students the opportunity to enroll full time at any of CCV’s 12 locations during the fall and spring of their senior year for a combined total of 8-10 classes (24+ credits). Credits apply to both high school completion and college. Classes may be online or in-person.

Fields housing discrimination complaints. People with credible issues covered by fair housing law are then referred to the Vermont Human Rights Commission, HUD, or Vermont Legal Aid, as appropriate. Also provides public advocacy, outreach, and education on fair housing issues, including publishing information, such as the Guide to Fair Housing for Families in Vermont, and conducting trainings and workshops about fair housing law.

Provides flexibly scheduled and routed door-to-door (or curb-to-curb) transportation services. Counselors and specialists in 12 district offices work together with an extensive network of partners to help participants assess their interests and skills, explore career options, obtain career-focused training, and secure jobs and career advancement.

Services include at-home assessments for expectant mothers and infants, postpartum assessments, newborn evaluations, breastfeeding support, nutrition education, parenting information, family adjustment support, skilled nursing care if needed, and assistance with coordination of family services.

Provides an online directory of mentoring programs throughout the state, searchable by county and by name of organization, with links to the individual websites.

Once registered, interested individuals will be notified immediately of any changes to an offender's custody status.

Offers a 12 Step recovery program for individuals who wish to stop the compulsive behavior of substance abuse. Also provides information about dates, times, and locations of meetings.

Provides Healthier Living Workshops for individuals living with chronic diseases and long term health issues to improve their health and well-being through participation in group exercise activities and health education classes.
