Provides the Vermont Deaprtment of Health Quit Time services that link Vermonters to free services and information to quit smoking. Offers phone, 802 QUITS, and in-person coaching, online support, access to free nicotine replacement (gum, patches and lozenges) and self-help tools, as well as to a Vermont Quit Partners Fresh Start Program series of weekly support meetings. Series is four-weeks long; new sessions start every month. Click on Support Group tab to see list of support groups. Project Independence Caregiver Support Group can provide respite care during meetings if it's necessary to bring a loved one. Contact Project Independence the week before to request.

INSURANCE CASE MANAGEMENT AND REFERRALS | VALLEY HEALTH CONNECTIONS

Provides assistance in navigating Vermont Health Connect, Green Mountain Care and FQHC health care systems to aid clients in obtaining free or subsidized health care through referrals, including: benefit screening and enrollment assistance, dental referrals and assistance in obtaining care, patient assistance and pharmacy programs, eye care referrals, tobacco use screening and cessation program referrals, and physician referrals. All referred services providing direct care are based on income eligibility. Outreach Referral Assistance appointments are available; drive-up appointments for patients with concerns or mobility issues are also available.

MY LIFE, MY QUIT | NATIONAL JEWISH HEALTH

Provides a free and confidential service for individuals who want help quitting all forms of tobacco, including vaping. Coaches help develop a quit plan, identify triggers for use, practice refusal skills, and provide ongoing support for changing behaviors. Also provides online resources for parents, guardians, educators, health care professionals, and community organizations, as well as free promotional materials.
