HELP ME GROW VERMONT | HELP ME GROW VERMONT

Operates a resource contact center that connects children and their families to services they need. HMG Care Coordinators provide education and support to families around developmental or behavioral concerns, help families recognize typical developmental milestones, offer confidential developmental screening, provide referrals to community-based supports, empower families to overcome barriers to services, and follow up with them to make sure linkages are successful. Help Me Grow also provides referrals to health and human services, including therapists. Help Me Grow Vermont provides the four components of the Help Me Grow System Model: -A centralized access point (in partnership with VT 211) that assists families and professionals in connecting children to the full assortment of community resources that help them thrive. -Family and community outreach that builds parent and provider understanding of healthy child development, the supportive services available to families in the community, and how both are important to improving children’s outcomes. -Child Health Care Provider outreach that supports early detection and intervention efforts and connects medical providers to the grid of community resources to best support families. -Establishment of an appropriate resource inventory and data collection and analysis that supports ongoing evaluation, helps identify systemic gaps, bolsters advocacy efforts, and guides quality improvement.

MENTAL HEALTH CARE AND SUBSTANCE USE DISORDER SERVICES | PEOPLE'S HEALTH AND WELLNESS CLINIC

Provides mental health and substance use disorder services that include psychiatry, clinical mental health counseling, screening for drug, alcohol, and tobacco use, screenings for depression, medications, prescriptions, and medication management, and referrals to specialists.

GIVE AN HOUR | GIVE AN HOUR

Provides referrals for free, confidential mental health services of an hour per week for a year. Give an Hour also supplies a variety of links to mental health information and resources for military families. Works to reduce the stigma associated with mental health problems through education, training, and outreach efforts. Also provides free webinars for Wellness Ambassadors for promoting and advocating for mental health awareness in communities. Telephone if without Internet service. Give an Hour does not provide emergency services. Mental Health Professionals who wish to volunteer must apply online, be licensed, and commit to providing free treatment for one year.

COMMUNITY HEALTH IMPROVEMENT | GRACE COTTAGE HOSPITAL

Provides holistic care services to all community members that may include the following Nursing, Behavioral Health and Physical Health Coaching services: Nutrition education, exercise education and counseling, short-term behavioral counseling, Diabetes education, individual health coaching, medication management, weight management-individual and group support, senior social isolation groups, Senior/ Disabled medical wellness home visits, smoking cessation, Advanced Directives, Fall Safety assessments, medical transportation and dental solutions. Grace Cottage's Community Health Team also provides a Resource Advocate who will assist with completing applications for the following services and assistance: health insurance (VT Health Connect and Medicare and Medicare D comparisons), SSDI/SSI, and prescription costs, as well as application assistance for housing, food, and utilities. Community Health Teams supplement the services available through primary care providers and serve to link individuals with the social and economic services that make healthy living possible for all community members. Each of Vermont’s Health Service Areas designs a Community Health Team to meet the needs of the local population and services may vary.
