Works with peers one on one and in groups to create independent living plans to reach an individuals' goals. Plans may include moving out of a nursing home, managing attendant care, budgeting finances, acquiring adaptive equipment, communicating more effectively, or finding a job.

Designed to support active military, veterans, and families of military/veterans with confidential, trauma-informed mental health care, provided by clinicians who understand the unique challenges faced by those who have served, including PTSD, transition stress, and substance use.
