Provides comprehensive services to individuals with developmental disabilities, autism, and other pervasive developmental disorders. Individuals are assigned professional service coordinators who are responsible for clients' Individualized Support Agreements, and for assisting them in achieving success in the community. Services include personal support and case management; specialized residential programs; psychiatric and medical care; vocational and skill acquisition training; family support services; and support services to individuals in nursing homes. Provides a spectrum of supported living arrangements to meet the needs of each person. Clinical services for individuals include provision of a psychiatrist to provide evaluation, medication checks, and after-hours consultation on medication issues. Also provides care coordination services. The Randolph office generally covers western Orange County, but service provision may overlap with the Bradford office area.

Educational resources include budgeting, employment, and application assistance. Case management and support s provided to youth and young adults who receive tenant based rental assistant with vouchers and administered monthly rental payments. Mediation is offered to support clients with communication with landlords, mitigating issues and concerns which arise to ensure secure housing for the youth and young adults.

Provides professional assistance with the assessment and development of plans for the care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for in-home care services. The Westminster community's Registered Nurse will coordinate the delivery of needed services, provide assistance/education around following medical advice and proper medication taking, assess in-home mobility safety to reduce the risk of falls, assist families with understanding the issues and needs of the family member requiring in-home care services, and provide ongoing follow-up visits to ensure the best possible health outcomes. All services provided are confidential and specific to each individual's needs.

Offers short term, holistic, intensive clinical case management, and social support services. Vermont Chronic Care Initiative (VCCI) licensed case managers meet with members in their homes to work on self-management plans. Case managers collaborate with providers and attend provider appointments to help facilitate collaborative care planning, meet clinical gaps in care. They also work with community/state agencies to address other needs, such as housing and transportation.
Provides service coordination, family supports, shared living arrangements, residential crisis services, respite support, community supports, employment supports, personal care services, medical coordination, psychiatric services, medical in-service and care-giver training, and financial management. Each person is served by a team that includes a service coordinator, nurse, guardian, support staff, and family members. Lincoln Street is a Specialized Service Agency, designated by Vermont's Department of Aging and Independent Living.

Provides in-home placement services for individuals who choose to live in a family setting rather than a larger residential care facility. Provides in-home support services to the individuals placed in the home care environment and to the individuals, couples or families hosting them. Home care services may include case/care management, nursing oversight, personal care assistance, medication management, transportation to doctor appointments, and day support. Host home providers are provided with a respite budget for periods of relief or rest from caregiving.

Provides eligibility screening for financial assistance programs. Offers personal assistance to qualifying individuals in obtaining ongoing health care with a doctor at reduced cost or for free. Offers assistance with obtaining affordable medications and eye glasses and accessing community resources and state programs. Also offers assistance with managing health concerns through case management, coordination of care, and communication with health care providers. Anticipate wait time for Health Assistance Program response due to high call volume. All calls are returned within two business days. Multiple calls by one caller to the HAP line does not help.
