The team also includes VT Health Connect navigators and assistors, as well as care managers for individuals who require support maintaining independence and stability within the community. Provides support services for patients and their families during hospitalization and upon discharge, for people receiving outpatient services, for previous patients and for other people not previously associated with the facility who need the service. Implements a Community Health Improvement Program based on the Community Needs Health Assessment.

Connects individuals who have abuse-related dental injuries with volunteer cosmetic dentists. They do not have volunteer dentists in Vermont but have volunteers in New Hampshire, New York and Massachusetts. The Give Back a Smile program does help with gas expenses for patients in the program traveling to their dentist.

Provides dental health care that includes the following: Oral health and oral hygiene education, on-site routine cleanings, sealants, topical flouride treatments, emergency evaluations, digital bite-wing and panoramic x-rays, full mouth deep cleanings, and cavity stabilization. Also provides dental care referrals to dentists, orthodontists, and periodontists.

All referred services providing direct care are based on income eligibility.
