Approved applicants may be granted up to $500 once per calendar year, which may be used for programs, services, and/or activities, such as exercise/wellness, medication, respite, assistance at home, transportation, and adaptive equipment. Awards are made on a first-come basis and are subject to the approval of an application and the availability of funds. Intended to support persons with Parkinson’s disease who are experiencing financial hardship and is not intended as an ongoing support fund. Payment may take at least 30 days to process after the application is approved.

Provides virtual education to individuals to help them manage blood sugar levels, learn more about different insulin options and devices, help with insurance issues, connect individuals to local resources, and prescribe needed drugs and devices. Offers a patient assistance program for individuals who cannot afford their medications and supplies and a Continuous Glucose Monitor (CGM) trial program for people that want to try managing their diabetes with the aid of a CGM.

Assists families with financial support for respite care and unmet medical expenses. Families may apply for a total of $1200 for Respite and/or Medically Necessary Funds per grant year.
