ALS INFORMATION, EDUCATION & SUPPORT SERVICES | ALS ASSOCIATION

Offers specialized information and referral to resources about living with ALS, including disability benefits and rights, insurance information, and home modification; ALS Equipment Loan Program focused on high cost durable medical equipment (mobility and ADLs) and augmentative communication needs; supports for newly diagnosed persons and caregivers; advocacy and education conferences; ALS Academy for professionals; information about research and clinical trials; and a newsletter. Equipment Loans: Monday-Friday: 9:00am-5:00pm; Days and Hours Vary for other services. Summer hours (Close at 3 pm on Fridays from Memorial Day to Labor Day)

FINANCIAL SUPPORT PROGRAM | AMERICAN PARKINSON DISEASE ASSOCIATION - VERMONT CHAPTER

Offers financial support to people with Parkinson disease and their families. Approved applicants may be granted up to $500 once per calendar year, which may be used for programs, services, and/or activities, such as exercise/wellness, medication, respite, assistance at home, transportation, and adaptive equipment. Awards are made on a first-come basis and are subject to the approval of an application and the availability of funds. Intended to support persons with Parkinson’s disease who are experiencing financial hardship and is not intended as an ongoing support fund. Payment may take at least 30 days to process after the application is approved.

Offers a complete line of home health care products and services, including respiratory and home medical equipment, stairway lifts and scooters, and specialty products such as back and leg braces. Provides home assessments, free equipment delivery, in-home set-up and training, and 24 hour on call service. Keene Medical Products will do the paperwork required for Medicare and other third party reimbursement.

Manages a home medical equipment loan and exchange program where individuals in need can either donate, borrow or exchange used in-home medical equipment that assists with daily living. Donated medical equipment must be in very good and clean condition.

ASSISTIVE TECHNOLOGY EXCHANGE PROGRAM | WAYWARD WHEELS

Provides inventory information that helps individuals and organizations donate and exchange usable assistive technology products such as wheelchairs, walking aids, standing aids, toilet aids, and other daily living aids . Maintains a wish list of items that individuals are looking for and assists with coordination of pick-ups as the items become available. Individuals should call to request an item before going to Wayward Wheels in person so they can prepare items in advance. Both offices are managed by individuals with disabilities who require assistance in preparing items to be ready for those in need. In the rare case, when picking up requested items is not possible, attempts will be made to assist with coordination of delivery.

VERMONT ASSISTIVE TECHNOLOGY PROGRAM | VERMONT ASSISTIVE TECHNOLOGY PROGRAM

Provides assistive technology (AT) device demonstrations, trainings, as well as information and assistance regarding AT devices and services, funding resources, and other disability-related topics. People can check out loans online, and have the item mailed to them. The program also offers public awareness via informational displays and equipment demonstrations at outreach events, and technical assistance regarding AT to agencies and programs.

VERMONT AT EXCHANGE | VERMONT ASSISTIVE TECHNOLOGY PROGRAM

Provides a way for individuals who have assistive technology and durable medical equipment items they no longer need to donate or sell them. Individuals who need items such as communication devices, wheelchairs, ramps, mobility devices, and vans can register with the exchange and view items as well as contact information for the owner of those items. This is an independent exchange between registered users. All equipment is held and transferred by owner and recipient; equipment is not held by the exchange itself.

UNITED HEALTHCARE CHILDREN'S FOUNDATION | UNITED HEALTHCARE CHILDREN'S FOUNDATION

Provides monetary grants of up to $5,000 (with a lifetime maximum of $10,000 per child) toward a family's share of the cost of medical services when gaps in a family's commercial health benefit plan mean that a child may go without necessary treatment, or when the child receives care and the family assumes a large amount of debt. Covered services include deductibles, co-insurance, co-pays, and services not covered by insurance. Provides a wide variety of mobility products including stair lifts (sales and installation), lift recliners, motorized scooters, power wheelchairs, manual wheelchairs, walkers, rollators, knee walkers, hospital beds, patient lifts and ramps for private homes, public schools or commercial enterprises. Also provides rentals of recliners, scooters, knee walkers, wheelchairs, hospital beds and ramps, and repairs may types of assistive equipment. Upward Mobility accepts Care Credit. They also work closely with organizations such as Agewell, the Vermont Center for Independent Living, Home Health and Hospice, and more to help secure funding for people with disabilities or those who are low-income.

DURABLE MEDICAL EQUIPMENT LOANS | BUGBEE SENIOR CENTER

Maintains a limited supply of small durable medical equipment to be loaned out as needed. Equipment includes bed assist bars and other supports; mobility assistance: canes, quad canes, and crutches, knee scooters, walkers, wheelchairs, and transport chairs; showering and toileting equipment; and tools for daily living assistance. Apply through the website. There is no limit on loan period. Donations of equipment in clean and good condition are accepted from the community. Call before coming in to donate equipment.
