Provides in-home assessments for fall risks by a community paramedic. Provides planning and basic home modifications such as grab bars, lights, railings, and tub mats to mitigate risks. This is a pilot program started in October 2023.

Makes recommendations for home modifications that can include ramps, grab bars, and stair adaptations. Services are located in the Medical Office Building of the Fanny Allen campus - Outpatient Rehabilitation, Suite 101 and 201. The street address is 792 College Parkway, Colchester.

Offers home inspections to assess the safety of the environment in which a minor child may be residing or visiting. Most often this service is requested by an agency, court, or custodial parent prior to approving in-home parent/child contact or overnight visits. Assessment may be tailored to the needs and concerns of the participants, court orders, and/or outside professionals.
