Topics include movement and exercise, nutrition, childbirth education, car seat safety, chronic disease self-management, and health/disease specific support group meetings. Provides the Vermont Department of Health Quit Time services that link Vermonters to free services and information to quit smoking. Offers phone, 802 QUITS, and in-person coaching, online support, access to free nicotine replacement (gum, patches and lozenges) and self-help tools, as well as to a Quit Time Tobacco Cessation series of weekly support meetings.

ADA / PARATRANSIT | GREEN MOUNTAIN TRANSIT

Provides transportation services for people with disabilities who are unable to use standard bus service. Service is provided within three-quarters of a mile of the fixed routes. Completed applications are evaluated within 21 days of receipt. Upon approval, individual can then contact Special Services Transportation Agency (SSTA) to make travel arrangements. Schedule by 4:30pm the day before travel.

Offers services and programs to eliminate barriers to obtaining healthcare such as: financial assistance applications for healthcare, dental care financial assistance for patients who meet criteria, help with enrollment and management of pharmaceutical programs for free medication and vouchers, and transportation vouchers when needed to get to healthcare or wellness appointments. Dental Care financial assistance is on a first come, first served basis with a limit of up to 1000 dollars for coverage

Provides preventive health care, as well as diagnosis and treatment of illnesses. Services include primary medical care such as physical health examinations, immunizations, acute and chronic health screenings, obstetric care, laboratory testing, and birth control counseling. Pediatric primary care is staffed by specialists who are concerned with the physical, emotional and social health of infants, children and adolescents from birth to young adulthood. Provides onsite integrated health services to enhance access to appropriate prevention, treatment, recovery and wellness services and activities.

TILLEY DRIVE SHUTTLE | SPECIAL SERVICES TRANSPORTATION AGENCY

Provides rides to a transportation hub, where riders can travel for any reason, including for health and wellness appointments. Provides a public transportation connection between the Downtown Transit Center (Cherry Street platform) and the offices at Tilley Drive off Hinesburg Road. Riders must provide location at Tilley Drive. Individuals traveling with a child up to age eight or weighing less than forty pounds must provide a child safety seat to ride in the shuttle.
