Offers a series of health-related workshops and presentations for the community, many of which are offered by health professionals. Information about local wellness programs is provided on line and is also published in flyer form. Topics include movement and exercise, nutrition, childbirth education, car seat safety, chronic disease self-management, and health/disease specific support group meetings. Provides the Vermont Department of Health Quit Time services that link Vermonters to free services and information to quit smoking. Offers phone, 802 QUITS, and in-person coaching, online support, access to free nicotine replacement (gum, patches and lozenges) and self-help tools, as well as to a Quit Time Tobacco Cessation series of weekly support meetings.

Provides wheelchair-accessible, door-to-door transportation to medical appointments and therapy, grocery shopping, adult day programs, senior meals, and personal and social activities. Door-through-door service includes assisting passenger within the origin or destination of their trip. Individuals not requiring wheelchair accessibility are usually transported in private autos operated by C.I.D.E.R. volunteer drivers. Most trips are "on demand" except for regularly scheduled shopping or excursion trips. May be able to accommodate early morning, evening and weekend requests as well.

Provides holistic care services to all community members that may include the following: Nursing, Behavioral Health and Physical Health Coaching services: Nutrition education, exercise education and counseling, short-term behavioral counseling, Diabetes education, individual health coaching, medication management, weight management-individual and group support, smoking cessation, Advanced Directives, Fall Safety assessments, medical transportation and dental solutions. Community Connections Community Health Team also provides Resource Advocates who will assist with completing applications for the following services and assistance: health insurance (VT Health Connect and Medicare and Medicare D comparisons), SSDI/SSI, and prescription costs, as well as application assistance for housing, food, and utilities. The team also includes VT Health Connect navigators and assistors, as well as care managers for individuals who require support maintaining independence and stability within the community. Provides support services for patients and their families during hospitalization and upon discharge, for people receiving outpatient services, for previous patients and for other people not previously associated with the facility who need the service. Implements a Community Health Improvement Program based on the Community Needs Health Assessment.

Programs for health maintenance and public health awareness services are also offered.
