SERVICES FOR OLDER ADULTS / PEOPLE WITH DISABILITIES | MOOVER, THE

Provides demand-responsive, door-to-door transportation for seniors and individuals with disabilities to senior meal sites, medical appointments including dialysis and chemotherapy, adult day care, employment-related appointments, social events, shopping, and other services and activities. Some trips are to scheduled destinations on set days of the week, while some trips can be tailored for individual needs. Some restrictions will apply.

BARNARD HELPING HANDS | BARNARD HELPING HANDS

Provides a variety of services to support the health and well-being of neighbors on a temporary basis. Services may include transportation, friendly visiting, certificates and forms assistance, yard work, assistance with errands, homemaker assistance, and minor home repairs and maintenance. Also provides one-time emergency financial assistance to individuals facing a short-term crisis as well as referrals to other agencies for more long term needs. Barnard Helping Hands in partnership with Barnard General Store maintains a food collection box to support Woodstock Community Food Shelf. They also work closely with the Hub in Woodstock, the Ottauquechee Health Foundation and the Woodstock Thomspon Senior Center.

ADA / PARATRANSIT | GREEN MOUNTAIN TRANSIT

Provides transportation services for people with disabilities who are unable to use standard bus service. Service is provided within three-quarters of a mile of the fixed routes. Completed applications are evaluated within 21 days of receipt. Upon approval, individual can then contact Special Services Transportation Agency (SSTA) to make travel arrangements. Schedule by 4:30pm the day before travel.

SERVICES FOR OLDER ADULTS / PEOPLE WITH DISABILITIES | SPECIAL SERVICES TRANSPORTATION AGENCY

Provides door-to-door transportation in lift-equipped vans to individuals unable to use public transportation. Eligible clients may use transportation for medical appointments as well as errands, shopping, and other non-medical appointments. Offers volunteer driving opportunities. Availability on a first-come, first-served basis. SSTA routes go 3/4 mile beyond Green Mountain Transit routes. Prospective volunteer drivers should contact the office for more information.

Provides transportation to senior meal sites and transports area residents to medical and employment-related appointments, social events, errand running, shopping, and other services and activities. Offers volunteer driving opportunities. May receive up to six free rides per month. Senior meals are not counted against monthly ride allowance. Provides transportation to and from medical appointments, senior center, shopping trips, and other essential travel needs. Appointment Required 24 Hours in Advance. The Castleton Community Center bus picks up eligible riders in Castleton, Hubbardton, Fair Haven, and Poultney.

MOUNTAIN TOWN CONNECTOR | NEIGHBORHOOD CONNECTIONS

Provides transportation to medical appointments and pharmacy visits. Transportation for other purposes includes but not limited to: food shopping, errands, social events, and access to Neighborhood Connections and community programming. Wheelchair accessible van is available. On request, driver or trained volunteer can accompany individual into their appointment or activity, for assistance or advocacy.

SENIOR RIDES AND MEDICAL TRANSPORTATION | C.I.D.E.R.

Provides wheelchair-accessible, door-to-door transportation to medical appointments and therapy, grocery shopping, adult day programs, senior meals, and personal and social activities. Door-through-door service includes assisting passenger within the origin or destination of their trip. Individuals not requiring wheelchair accessibility are usually transported in private autos operated by C.I.D.E.R. volunteer drivers. Most trips are "on demand" except for regularly scheduled shopping or excursion trips. May be able to accommodate early morning, evening and weekend requests as well.

WESTMINSTER CARES PROGRAMS | WESTMINSTER CARES

Provides programs that help individiuals to remain in their homes and connected to the community. Services may include transportation to medical appointments, shopping, friendly visits and reassurance calls, chores and yard work, and homemaker assistance. Also provides medical equipment loans, as well as a modest home heating assistance fund. The only wheelchair-accessible entrance to Westminster Cares is the back entrance.

THOMPSON TRANSPORTATION PROGRAM | THOMPSON SENIOR CENTER

Provides local transportation to and from the Senior Center, West Lebanon shopping trips, and other essential travel needs. Offers volunteer driving opportunities. Reservations for rides to and from Thompson Senior Center and Woodstock required by 8:30am. Medical rides must be scheduled a minimum of 48 hours in advance. Transportation is also available, with advance notice, within Woodstock for errands.

ACCESS AT | ADVANCE TRANSIT

Provides curb-to-curb transportation services for individuals who are unable to use fixed-route public transportation due to a disability. Serves individuals within 3/4 mile of the fixed-route service. Applicants will be notified of their eligibility within 21 days of assessment. Application is available for download from the website.

To become a volunteer driver, contact Tri-Valley Transit or visit website. The Dial-A-Ride Program provides four free rides per month per eligible rider.
