CHOICES FOR CARE | DAIL - ADULT SERVICES DIVISION
Provides a package of long-term services and supports to Vermonters who need nursing home level of care. Eligible persons choose where to receive their services - in their home, their family’s home, an Adult Family Care Home, Enhanced Residential Care, or nursing facility. Services include personal care, respite care, companion care, adult day services, assistive devices, home modification, personal emergency response system, and case management. Enhanced residential care services include nursing overview, personal care, medication management, social and recreational activities, 24-hour on-site supervision, laundry services, household services, transportation, meals, and case management.
