DISABILITY DETERMINATION SERVICES | VERMONT DEPARTMENT FOR CHILDREN AND FAMILIES
Provides disability determination services for applicants who have applied for Social Security Disability Insurance (SSDI), Supplemental Security Income (SSI), and disability-based Medicaid. Services are provided in accordance with Social Security federal statutes, regulations, and policy. If you disagree with the decision on your disability claim, do not use this process. Follow the instructions in the determination notice you received.

Application process
Applications for Medicaid can be obtained from and are first filed with the Vermont Department of Health Access (DVHA). Applications for Social Security and SSI Disability must be filed with the Social Security Administration.

Vermont residents, children and adults, who have applied for Social Security, Supplemental Income (SSI) or Medicaid on the basis of disability; In order to establish eligibility, Disability Determination Services contacts your healthcare provider to obtain the detailed medical and functional information needed to document each disability claim.
