Vermont state
Provides eligibility screening for financial assistance programs. Offers personal assistance to qualifying individuals in obtaining ongoing health care with a doctor at reduced cost or for free. Offers assistance with obtaining affordable medications and eye glasses and accessing community resources and state programs. Also offers assistance with managing health concerns through case management, coordination of care, and communication with health care providers.
