HOME NURSING / HOME HEALTH CARE | VISITING NURSE ASSOCIATION AND HOSPICE OF THE SOUTHWEST REGION
Provides occasional nursing and rehabilitative therapies in the patient's home. Skilled Nursing Services include nursing care, case management, and client and family education. Provides clients with social workers who offer coordination of community services and resources, client and family counseling, and navigating the health care system. Licensed Nursing Assistants provide home health assistance, including personal care assistance, light housekeeping, and help with normal daily living activities.

The Visiting Nurse Association & Hospice of the Southwest Region is a non-profit, Medicare-certified, home health care and hospice agency delivering services to residents of Southwestern Vermont and Franklin County. Care is provided to adults who need in-home assistance, rehabilitation, long term care, or end-of-life care, as well as to critically ill children and to vulnerable young families. The Agency promotes the general welfare of residents through health promotion outreach and community wellness programs. Services include Home Care, Hospice Care, Palliative Care, Long Term Care, Rehabilitative Therapies, Maternal Child Health Services, Pediatric Nursing, and Private Duty Nursing Services.
