Provides physical health exams, acute and chronic health care, immunizations, women's health screenings, limited mental health counseling by appointment, lab and blood work, and assistance with finding affordable prescription medications. Provides patient-centered physical and mental health care delivered by a team of primary care and mental health clinicians .The clinics also provides referrals to other health and non-health services, and will screen patients and callers for eligibility for other health services and public insurance programs, as well as assisting with the application process. Also hold diabetes education classes. Patients must apply for Medicaid, if applicable; Patients may be seen at Express Care at the Castleton and Rutland Locations;
