Provides patient-centered physical and mental health care delivered by a team of primary care and mental health clinicians. Offers special services for at-risk youth, homeless teens, and young adults. Services include confidential STD and pregnancy testing and free/low cost medications. Also provides consultations and support for eligibility in State and Federal Benefits.

The Community Health Center of Burlington (CHCB) is a non-profit, community-based Federally Qualified Health Center (FQHC) offering comprehensive primary health care services to people of all ages, backgrounds, and life circumstances. CHCB provides primary medical and dental care as well as mental health and psychiatry services. Special services include telephone-based interpreter assistance, obstetrical and prenatal care, prescription assistance and a sliding fee scale.
