Service Hours: Monday-Sunday: Transportation is scheduled as needed

- Referral may be required from partnering human services agency, including Northern Counties Health Care, Northeastern Vermont Regional Hospital, Northeast Kingdom Council on Aging, Northeast Kingdom Human Services, Adult Day Health Services, Northeast Kingdom Community Action Agency, or Central Vermont Council on Aging
