Description

Provides nurses and/or other trained professionals who visit the homes of women during pregnancy and after childbirth to help women improve their personal health and development during the prenatal period and learn to care for their infant. Services include at-home assessments for expectant mothers and infants, postpartum assessments, newborn evaluations, breastfeeding support, nutrition education, parenting information, family adjustment support, skilled nursing care if needed, and assistance with coordination of family services.

The Visiting Nurse Association & Hospice of the Southwest Region is a non-profit, Medicare-certified, home health care and hospice agency delivering services to residents of Southwestern Vermont. Care is provided to adults who need in-home assistance, rehabilitation, long term care, or end-of-life care, as well as to critically ill children and to vulnerable young families. The Agency promotes the general welfare of residents through health promotion outreach and community wellness programs. Services include Home Care, Hospice Care, Palliative Care, Long Term Care, Rehabilitation Therapies, Maternal Child Health Services, Pediatric Nursing, and Private Duty Nursing Services.
