Provides clients with social workers who offer coordination of community services and resources, client and family counseling, and navigating the health care system. Licensed Nursing Assistants provide home health assistance, including personal care assistance, light housekeeping, and help with normal daily living activities.

The Visiting Nurse Association & Hospice of the Southwest Region is a non-profit, Medicare-certified, home health care and hospice agency delivering services to residents of Southwestern Vermont and Franklin County. Care is provided to adults who need in-home assistance, rehabilitation, long term care, or end-of-life care, as well as to critically ill children and to vulnerable young families.
