Description

Provides comprehensive primary and preventive health care, including on-site behavioral health professionals. Services include medical, mental health and substance abuse counseling, family planning, and lab services. The Community Health Pharmacy program offers discounted prescriptions to all patients. Also provided is eligibility support for State and Federal Benefits and low-cost drug manufacturer giving programs. Confidential HIV Testing available at every affiliated community health center. Patients of Champlain Islands Health Center can access dental care at the South End Health Center in Burlington.

The Community Health Center of Burlington (CHCB) is a non-profit, community-based Federally Qualified Health Center (FQHC) offering comprehensive primary health care services to people of all ages, backgrounds, and life circumstances. CHCB provides primary medical and dental care as well as mental health and psychiatry services. Special services include telephone-based interpreter assistance, obstetrical and prenatal care, prescription assistance and a sliding fee scale.
