Provides patient-centered physical and mental health care delivered by a team of primary care and mental health clinicians. Services include medical, mental health and substance use disorder counseling, psychiatry, family planning, dietary counseling, Sexually Transmitted Infection (STI) screening, and on-site lab services. The Community Health Pharmacy program offers discounted prescriptions to all patients. Also provides consultations and support for eligibility for State and Federal Benefits and low-cost drug manufacturer discount programs.

The Community Health Center of Burlington (CHCB) is a non-profit, community-based Federally Qualified Health Center (FQHC) offering comprehensive primary health care services to people of all ages, backgrounds, and life circumstances. CHCB provides primary medical and dental care as well as mental health and psychiatry services. Special services include telephone-based interpreter assistance, obstetrical and prenatal care, prescription assistance and a sliding fee scale.
