MODERATE NEEDS GROUP | DAIL - ADULT SERVICES DIVISION
Provides limited funding for homemaker, adult day, case management, and ‘flexible fund’ services to people who do not meet nursing home level of care criteria, but require some services to assist them to remain independent in their home, preventing a more intense level of service. The intent is to prevent or delay the need for more costly long-term services and supports by providing these services. The Moderate Needs services are limited by available funds.

By Telephone; Applications are to be mailed to the Case Management Agency of choice from among those in your area, which are listed on the back of the application. Case Manager will complete an assessment, as well as clinical and financial eligibility screenings
Eligibility
Adults Age 18 and Older who have functional limitations related to a physical condition (including, but not limited to stroke, dementia, and traumatic brain injury), or associated with aging; Income Guidelines Apply; Must meet eligibility criteria for Instrumental Activities of Daily Living (IADL) determined through a case management agency assessment.
